In the Infant room a roll of paper towels sitting on a shelf and not in a dispenser which is unsanitary. Outlets are uncovered in the 1’s-2’s room, School-age room and dinning area which is unsafe. In the dinning area there is a hole in the drywall from the door handle which is unsanitary.

In the Infant room the seal around the sink is broken which is unsafe. Roll of paper towels uncovered sitting on a box which unsanitary. Infant room, PreK room, 2’s, and 1’s rooms have peeling paint and dents in the wall which is unsanitary. In the 1’s room the black rubber carpet seal is loose and a tripping hazard. In the 2’s room there are 2 ceiling tiles with water stains and the air vent is dirty which is unsanitary. The 4’s room floor tiles are broken which is unsanitary and the gold metal threshold is broken which is a tripping hazard. In the 3’s room there is Shaving cream on the toy shelf accessible to children which is unsafe. In the 3’s room the dark green wall has a dent exposing drywall which is unsanitary.
