    On April 1st, 2026, the child care center failed to ensure that children were under direct supervision at all times. A self-reported indicated that one staff member was seated on the carpet reading to a group of children, while several other children were engaged in table activities. A second staff member was assigned to supervise restroom which was connected to another classroom. During this time, the staff member turned away from the restroom area to assist another child who was attempting to climb a latter being used by maintenance staff changing a light bulb. While the staff member's attention was diverted and her back was turned, children in the restroom area of the connected classroom were not directly supervised. As a result, an inappropriate interaction occurred between them.
