Room 12 Floor has moisture on it and can be seen- staff member slipped when going in - white plastic bucket on counter mold, ledge of bathroom mold spots. Small humidifier in bathroom on shelf that drains into handwashing sink of bathroom. Room 1 chairs still in place that had mold before and can see more mold on them. Room 9 Infant room both chairs had crib sheets over them. On one near south wall can see the mold on the back of the chair. Took cover off both chairs to see if more- none seen. Has infants in room Room 2 - Floor sq carpet pulled back and has mold on it and the floor had mold spots. Room 10 - 1 crib near back room can see mold on wood and in cracks - picture taken - also the baby changing pad gray and sink mold trim show mold spots Room 3 - Diaper table pad - books - climb over - and trash can all show mold spots - trash can lid was gross Room 4 - humid and humidifier read 69 mold sink trim this room has Toddlers in it Room 8 - Big dehumidifier read 75 has multiple carpet squares laid out seeing mold spots and stains and other materials in room that did not go through. Looks like items pulled from classrooms. Room 6 - moisture floor- humid 68 on humidifier -

The readings off temperature settings in office read 66 to 70 degrees. As walked through rooms could feel some rooms hotter and some rooms colder. Their is no consistency in temperature.

Temperature was inconsistent throughout facility and not maintained at 68 degrees F. Some rooms hot and muggy, others cold and damp, one room had condensation build up on floor and consultant almost fell.
