    470 IAC 3-1.2-5(a) · Licensee shall provide a separate crib, portacrib, or playpen in which each infant or toddler can sleep. Each crib, portacrib, or playpen shall be equipped with a firm-fitting mattress or pad made of waterproof materials. A parent or legal guardian may provide or approve the use of a bassinet for an infant and is responsible along with the caregiver to monitor its use closely. (Manufacturers of bassinets indicate that a bassinet should no longer be used once an infant begins moving and turning unassisted.)

    At today's visit, a child who is 8mo was alseep in a crib with a quilt pad underneath him. This pad was not the mattress cover, but extra soft bedding. Nothing extra(blanket, bottles, bumpers, etc) can be in a pack and play/crib with a infant under 12 months. Pad was moved when noted.

    At today's visit there were 3 separate safe sleep non-compliances viewed. One 11 month old was asleep in a crib with a bottle. The second child was 6 month old and was asleep in a car seat with a blanket covering it. The car seat was in a crib. The third child was said to be 5 months old, he was alseep in a baby swing. Consultant had the provider move all children.
