Program self reported two separate incidents concerning supervision on the playground. Videos support that the caregivers in one incident sat in chairs and did not interact with children in all areas of the playground or make sure that all children were visible before leaving a child on the playground. In the second incident the caregiver did not ensure visibility of the students shaking/escaping from the gate or move from a bench on the playground to view children.

Program self reported two separate lack of supervision incidents: On 4/14/26 a three year old child was left unattended on the playground for ten minutes. On 4/15/26 five children shook open locked playground gate and exited the playground, were unsupervised for over one minute outside of the playground. At time of consultant's visit today a large floor mat was leaning against the end of a crib, blocking infant staff's view of child that was in crib.
