The kitchen counters were dirty and appeared to have dirt accumulated on them. The food preparation areas had dust and dirt on the counter by the microwave, stove and dry food storage areas. The kitchen floors were dirty with what looked like dried up dirt and other substances that could have been food or liquids dried on it.

470 IAC 3-1.1-45(a) 25a: The home must be maintained in a clean, sanitary, and safe condition: The bathroom of the home daycare was not clean and sanitary. There were two seat coverings for the toilet seat in the bathroom. The large one was on the toilet seat and was dirty. Consultant was informed by a staff member that a child that attends the daycare requires the special toilet seat. However, the child was not in attendance on the day of inspection and the toilet seat was too large for the smaller child that was in attendance. The small one for potty training was sitting on the floor and was also dirty. The bathroom floor and sink were dirty and in need of cleaning. The floor had a wet and dry substance on it that was sticky to the bottom of Consultant's shoes. There were pieces of toilet paper on the floor in multiple places on the bathroom floor that were "wadded up". The home had ceiling tiles that were loose and sagging that could potentially fall. They were not secured. The ceiling in the bathroom had a place cut out of it above the washer and dryer that exposed the outside of the home through a crack in the roofing/wall inside of the ceiling cut out. The bathroom ceiling paneling was loose and hanging on the right side of the bathroom, above the sink and diaper chaning table, and was not secure and could fall.

The wood decking outside of the back door in the playarea has loose boards, missing pieces of boards and there are nails in the boards. This could cause harm or injury to a child that walks on or plays on the decking.
