470 IAC 3-4.5-5(b) · Food Safety. All foods provided by the facility, for children enrolled in the day care ministry, shall be from a food establishment, inspected and approved by a governmental agency. Food items shall be received at the facility in the original, unopened, undamaged packaging and shall be properly protected from damage and potential contamination. Food shall be free from spoilage, filth, or other contamination and shall be safe for human consumption. The temperature of all potentially hazardous food shall be 45 F. or below or 140 F. or above at all times. Frozen food shall be kept frozen and should be stored at a temperature of 0 F. or below

When consultant entered room a 5 month old male was asleep in an activity bouncer. There was also a 3 month old female asleep in a crib laying on an activity mat. There was also a sound machine on the side of the crib with the 3 month old female.
