    470 IAC 3-4.7-94(r) · Caregivers shall properly cleanse the childs body at each diapering. The submitted health program shall contain skin care procedures approved by the centers health consultant. Caregivers shall post and follow the procedures when diapering children

    Incident was on 10/2/25 and reported on 10/3/2025 as a complaint. A 3 year child was not properly cleansed during the potty process. The child is not potty trained and from lack of proper cleaning had redness and rash per report. The Assistant Director stated staff was retrained and they believe they have identified the time that the issue was happening. The child is still enrolled.
