During an Incident Investigation Closure conducted on January 21, 2025 ; It was determined based on an investigation that on November 21, 2024, at approximately 4:30 p.m., a two-year-old child was left unsupervised on the playground for an undetermined amount of time. There was one staff member and eight children assigned on the playground at the time of the incident. The staff member did not provide adequate supervision when they left the two-year-old child on the playground alone for less than 30 minutes, but more than five minutes when the temperature was 54 degrees Fahrenheit. Professional medical attention was not obtained.

During a Licensing Study conducted on November 18, 2025 ; It was determined based on observation that a child from Room D - Toddler 2 was found standing in the hallway by the Consultant when they exited the kitchen. The child was unattended and all classroom doors were closed. Exhibit "A"

It was determined based on an investigation that on November 21, 2024, at approximately 4:30 p.m., a two- year-old child was left unsupervised on the playground for approximately five to thirty minutes in 54-degree Fahrenheit weather. The child was discovered by another parent who was picking up their child as they were leaving the center. The parent returned to the center and reported it to staff. The parent proceeded to open the door to the playground to retrieve the child. The department determined that the staff member did not provide adequate supervision when they left the two-year-old child on the playground alone.

It was determined based on an investigation that on June 26, 2024, at approximately 4:30 pm, in the Toddler 1 classroom, a one-year-old child sustained an injury to their head. A one-year-old child had blood on the back of their t-shirt and a small bump on the back of their head. It was determined that staff members were not aware how the injury occurred. The investigation determined that staff failed to provide watchful oversight in order to prevent the injury from occurring. Professional medical attention was received.

During an Incident Investigation/Monitoring visit, conducted on May 11, 2022, it was determined that a staff member was previously observed by other staff members to roughly handle children by hitting them when trying to direct them to nap or to sit down. A staff member that observed the inappropriate discipline reported it to the center director.
