591-1-1-.15(9) · Rule 591-1-1-.15(9) requires Modified Diets. When a child requires a modified diet for medical reasons, a written statement from a medical authority shall be on file. When a child requires a modified diet for religious reasons, a written statement to that effect from the child’s Parent(s) shall be on file. All caregiver Personnel shall be informed of the diet restriction for the child and only food that complies with the prescribed dietary regimen but still meets the food and nutrition requirements shall be served to the child.

It was determined that on April 19, 2023, at approximately 11:00 a.m., a one-year-old with a known allergy to eggs was served eggs during lunchtime. The two staff members in the classroom had previously been made aware of the child’s egg allergy and there was also a sign posted in the classroom with the child’s name and their known egg allergy. The child had an allergic reaction and medical attention was obtained.
