During a Licensing Study conducted on November 13, 2023 ; It was determined based on a review of transportation records that eight (8) out of thirty-two (32) children were missing the emergency medical information forms.

During a Monitoring Visit conducted on April 10, 2024 ; It was determined based on review of transportation records that eight (8) out of twenty five (25) children that were being transported were missing the emergency medical information form that is required to be completed by the parent.

During a Licensing Study conducted on November 13, 2023 ; It was determined based on a review of transportation records that children were not marked for loading and unloading for transportation to C.B. Watson Elementary. On October 30, 2023, two (2) of ten (10) children were not marked for loading and unloading. In addition, on October 31, 2023 through November 2, 2023, one (1) of ten (10) children was not marked for loading and unloading.

During a Licensing Study conducted on June 21, 2023 ; It was determined based on observation, that weeds and thorny vines were growing through the fencing to the left side of the playground, behind the older children area. Also, an old refrigerator was observed sitting on the playground.

During a Monitoring Visit conducted on April 10, 2024 ; It was determined based on observation the following hazards: * Weeds and branches growing through the fencing of the back left corner of the left side of the fence. * Three active ant beds, one located underneath the left side of the storage building and two located at the bottom of the poles to the tic- tac toe play equipment. * A mop and mop bucket located outside of the infant classroom door * A high-chair and two infant swings were on the playground near the door of the infant classroom. * A garden rack located at the front corner on the left side of the fencing.
