During a Complaint Investigation and a Licensing Study conducted on March 6, 2023; During a Complaint Investigation & Licensing Study conducted on March 6, 2023 ; It was determined based on review of transportation records that during the week of February 13, 2023, a first check was not conducted on February 16th and 17th during the morning and afternoon bus routes to and from Westside Elementary. In addition during the week of February 13, 2023, on February 13th, 14th, 15th, 16th and 17th, a first check was not conducted during the morning and afternoon routes to and from Parkwood Elementary. During the week of February 27, 2023, on Friday, March 3, 2023, a first check was not conducted during the afternoon routes from Parkwood Elementary, Westside Elementary, Pearl Stephens Elementary and C.B. Watson Elementary.

During a Monitoring Visit conducted on December 14, 2022; During a Monitoring Visit conducted on December 14, 2022 ; It was determined based on review of transportation records, there were no records for consultant to view during the month of December so consultant reviewed the transportation checklist during November 14th through November 18th. During the week of November 14th, the morning bus route for Northside Elementary School, on November 16th, 17th, and 18th five out of five children were missing a mark indicating loading at the facility and unloading at the school. In addition, during the week of November 14th, the afternoon route for Northside Elementary school on November 16th, 17th and 18th five out of five children were missing a mark indicating loading at the school and unloading at the facility. Also, during the week of November 14th, for the morning bus route to Westside Elementary School on November 17th, one child out of one child was missing a mark indicating unloading at the school and on November 18th one child out of one child was missing a mark indicating loading at the facility and unloading at the school. In addition, during the week of November 14th for the afternoon bus route from Westside Elementary School, one child out of one child was missing a mark for loading at the school and unloading at the facility.

Watson Elementary. 5. A. Rule 591-1-1-.36(7)(c )3. requires that the driver or other designated person document in writing the time of arrival and departure each time the vehicle departs from the Center, is loaded or unloaded at each school and when the vehicle returns to the Center; each time the vehicle departs from the Center, arrives at the location where any child is picked up or dropped off and when the vehicle returns to the Center; each time the vehicle leaves the Center, arrives at a field trip destination, leaves a field trip destination, and returns to the Center. During a Monitoring Visit conducted on December 14, 2022; During a Monitoring Visit conducted on December 14, 2022 ; It was determined based on review of transportation records that during the week of November 14th, on November 16th, 17th and 18th on morning and afternoon bus routes to Northside Elementary and Westside Elementary, arrival and departure times each time the vehicle departed from the center, arrived at the location of the school and when returning to the center were not recorded on the transportation form for each route. 5. B.
