During a Monitoring Visit conducted on January 31, 2022 ; It was determined based on observation that there was a four(4) inch gap at the bottom of the black iron fence, a five(5) inch gap at the bottom of the chain link fence near the basketball pole, and an eight(8) inch gap at the back right corner on Playground B. Further, the gate on the back left side was not locked allowing children access to air conditioner units, wood, and equipment on Playground B. A four(4) inch gap was observed at the bottom of the middle black iron fence on Playground A.

During a Monitoring Visit conducted on January 31, 2022 ; It was determined based on observation that children had access to exposed tree roots beneath the slide and the ground was not flushed with the concrete at the corner of the building and around the classroom doors on Playground B. The concrete measured between three (3) and four (4) inches high from the ground posing a potential tripping hazard. In addition, the consultant observed exposed tree roots at the back left of the playground beside the fence and in the middle of the playground near the play equipment on Playground A. Two white caps connected to the water system were protruding out of the ground posing a potential tripping hazard on Playground A.

Further, concrete at the corner of the building on Playground B was not flushed with the ground.
