    During an Incident Investigation Closure conducted on July 21, 2025 ; It was determined based on an investigation that on May 27, 2025, around 4:41 p.m., center staff was observed on video footage to roughly handle a 2-year-old child by grabbing them and sitting them down on a carpet as a form of discipline.

The outdoor equipment shall be free of lead-based paint, sharp corners and shall be regularly maintained in such a way as to be free of rust and splinters that could pose significant safety hazard to the children. All equipment shall be arranged so as not to obstruct supervision of children.

    During a Monitoring Visit conducted on November 24, 2025 ; It was determined based on observation that the middle playground gym had exposed rusting from the metal sides falling off on the staircase that posed a cutting hazard.

    591-1-1-.03(9) · Rule 591-1-1-.03(9) requires that staff shall not engage in, or allow children or other adults to engage in, activities that could be detrimental to a child’s health or well-being, such as but not limited to, horse play, rough play, wrestling, and picking up a child in a manner that could cause injury.

    During an investigation, it was determined that on May 27, 2025, a staff member allowed children to open and close a bathroom door repeatedly, resulting in a thumb injury for a 2-year-old child. The staff member was caring for two 1-year-old children and one 2-year-old child when the incident occurred. Based on video footage, the staff member was helping the 2-year-old child in the bathroom while the two other children were playing with the bathroom door, opening and closing the door repeatedly. After the 2-year-old child finished in the bathroom, they stood near the door, placing their fingers near the doorframe. The bathroom door was again closed, and the child’s finger was injured. The child cried out and the staff member picked the child up, then sat them on the rug with the remainder of the children. Once the staff member realized the child was injured, first aid was administered, and the child’s parents were called. The injury required professional medical attention.

    During an investigation, it was determined that on May 27, 2025, a staff member used inappropriate discipline when a 2-year-old child was roughly handled. A group of three children were repeatedly opening and closing the bathroom door in the classroom, resulting in a finger injury for the 2-year-old child. When the injured child cried out, the staff member grabbed the child forcefully by the arm, placing them on the rug in the classroom. The child showed their finger to the staff member, who grabbed the child by the arm and walked them to the sink to administer first aid.

    591-1-1-.12(1) · Rule 591-1-1-.12(1) requires all indoor and outdoor furniture, activity materials, and equipment shall be used in a safe and appropriate manner by each Employee and child in attendance and in accordance with the manufacturer's instructions, recommendations, and intended use. All equipment and furniture shall be used only by the age-appropriate group of children.

    During an investigation, it was determined that on April 7, 2025, a staff member failed to fasten a 5-month-old infant into a highchair resulting in the child falling out, face first, and hitting the floor. The staff member put the child in the high chair and did not check to ensure the tray was fastened securely. When the child leaned forward to reach for a toy the tray came off and the child fell face first onto the floor. Additionally, the safety straps were not attached to the highchair for use due to them being in the laundry. The child sustained a head injury that required professional medical attention.

One staff member was working in the classroom when they were removing trash from the classroom. The staff was unaware that the child was near the door and the child’s finger was closed in the door, resulting in a severed finger. The child required professional medical attention as a result of the incident.

The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm’s length away from children thirty-six (36) months of age and younger.

    During an investigation, it was determined that on May 17, 2021, a three-year-old child was missing from the center for an undetermined period of time when the child left the center and was found walking on the side of the road by a stranger. The child left the classroom and walked out of the center while the staff member left the classroom unsupervised to use the restroom. The staff member returned back to the classroom and noticed that the child was not in the room. Several passersby stopped to assist the child and to retrieve him from the road. One passerby went to the child care learning center to ask if a child was missing and Law Enforcement was notified. There were no visible injuries and medical attention was not obtained. The child was released into the care of his parents.
