It was further determined that in Main Room A, a thin electrical wire for lighting was dangling over the edge of one infant's crib that was not occupied at the time of observation.

Additionally, three other children were transported during the current week whose emergency medical form did not include the medical facility used by the center.

During a Licensing Study conducted on March 25, 2025 ; It was determined based on observation that the following hazards were observed to be present on the playground: -The exterior door to the after school room had splintered wood along the door frame and exposed foam underneath. -On the playground to the left, exposed rebar underneath the green and yellow structure with a blue slide posed a tripping hazard and had sharp edges. -On the playground to the left, there was exposed landscape fabric underneath the blue ABC play structure that posed a tripping hazard. -Trash that included styrofoam cups, food wrappers, and paper towels. -AC unit gate latch was not secured. -The silver horse play structure had exposed concrete underneath. -On the playground to the left in the back right corner there was exposed concrete that had sharp edges and posed a tripping hazard. -On the playground to the left there was a large section of PVC pipe along the base of the building with sharp edges on the end.
