During an investigation, it was determined that on August 22, 2025, the second check of the vehicle was not completed resulting in a 4-year-old child being left unattended on the center vehicle and outside of the Pre-K building for approximately 50 minutes. The transportation vehicle arrived at the center at 3:18 p.m., staff exited the vehicle at 3:19 p.m., walked to place their personal items in their personal vehicle, and then entered the Pre-K building. The transportation documentation indicated that the second check was completed by the driver at 3:20 p.m. Video footage showed the child exiting the vehicle at 3:57 p.m. and appeared to skip to the Pre-K building. Center staff failed to physically check underneath the seats, to ensure that there were no children left in the vehicle. The center staff failed to complete the second check of the vehicle which resulted in the 4-year-old child being left unattended in the vehicle and outside of the Pre-k building for 50 minutes.

During an investigation, it was determined that on August 22, 2025, a 4-year-old child was left unattended in a vehicle after the vehicle returned to the center following transporting children home. According to staff statements, there were two staff members transporting five Pre-K aged children home after school. The transportation document listed the child as absent although the child entered the vehicle. Video footage showed that at 3:18 p.m. the vehicle arrived back at the center. When the vehicle returned to the center, the center staff failed to ensure that the child exited the vehicle. The 4-year-old child was left unsupervised in the vehicle. At 3:57 p.m., the 4-year-old child exited the vehicle, went across the parking lot, and went to the Pre-K building and attempted to gain entry. The child was located outside of the Pre-K building by a staff member at 4:08 p.m. The staff failed to provide adequate supervision and watchful oversight when the staff members were unaware that the 4-year-old child was missing and unsupervised for 50 minutes.

During a complaint closure conducted on April 25, 2023, it was determined on February 24, 2023, center staff failed to provide adequate supervision when a staff member left a seven-year-old child alone and unattended on the center's transportation vehicle for two minutes. The child was picked up from WG Nunn Elementary school at 3:15 p.m., and the vehicle arrived at the center at approximately 3:20 p.m. Center staff unloaded the children and parked the vehicle. The staff did not complete a thorough check of the vehicle and went inside. Approximately two minutes later, another teacher walked outside to their vehicle, which was parked next to the center vehicle, and saw the child standing on the bus and went to assist. The child was escorted inside and checked for injuries and then taken to the after-school program. The center attempted to report the incident to the child’s parent by phone but were unable to reach the parent. The parent was notified at pick up at 5:55 p.m.

The seven-year-old child was left unattended on the vehicle for approximately two minutes. The staff member responsible for the first check acknowledged that the vehicle was not checked.

Approximately two minutes later, the child was discovered by an after-school teacher who had walked to their vehicle which was parked by the center vehicle. The staff member retrieved the child from the vehicle and walked the child to the after-school classroom. The seven- year-old child was left unattended in the vehicle for approximately two minutes. 4. The driver or other designated person shall immediately document in writing, with a check or other mark/symbol to account for each child listed on the passenger transportation checklist each time a child enters and exits the vehicle. The driver or other designated staff person shall document in writing with a different mark/symbol to account for each child listed on the passenger transportation checklist who was not present on the vehicle for any reason. An explanation shall be documented in writing whenever a child is transported to a field trip site but is not present on the return trip to the Center. During a complaint closure conducted on April 25, 2023, it was determined that on February 24, 2023, the center conducted routine afternoon transportation from WG Nunn Elementary School to the facility. Upon arrival at the center, the driver documented the checklist to indicate that all 15 children had exited the vehicle when one child remained on the bus. No first or second check of the vehicle was conducted. Another staff member found the child approximately 2 minutes later.
