During a Licensing Study conducted on August 17, 2023 ; It was determined based on a review of transportation records for the current week that four of twelve children transported from school to the center in the afternoon did not have the required emergency medical information on the vehicle used to transport them. Child No. 5 and Child No 6 were transported from W. G. Nunn Elementary School to the center on August 14, 2023, August 15, 2023, and on August 16, 2023, with no emergency medical information on the vehicle. Child No. 4 was transported from W. G. Nunn Elementary School to the center on August 14, 2023, and did not have emergency medical information on the vehicle while he was transported. Additionally, Child No. 3 was transported from Westside Elementary School on August 15, 2023, and on August 16, 2023, and did not have emergency medical information on the vehicle when she was transported.

During a Complaint Investigation & Monitoring Visit POI Follow Up conducted on April 11, 2024 ; It was determined based on a review of records that Child # 2 and Child # 3 were transported from W. G. Nunn Elementary School to Jackson Heights Christian Academy on the afternoon of April 10, 2024, with an incomplete emergency medical form on the vehicle. The forms for these two children did not include the name of the children's doctor and telephone number or the medical facility used by the center. Additionally, it was determined based on a review of records that on the mornings of April 9, 2024, and April 10, 2024, Child # 1 was transported from her home to Jackson Heights Christian Academy with no Emergency Medical Form on the vehicle.

During an Incident Investigation Closure conducted on February 1, 2024 ; It was determined based on an investigation that on December 6, 2023, at 7:42 a.m., the staff member signed the checklist indicating that the first check had been conducted, but did not physically walk through the entire vehicle's interior to ensure all the children had exited the vehicle. As a result, a four-year-old child was left unattended on the vehicle for approximately three minutes An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an investigation, it was determined that on December 6, 2023, a second check of the vehicle was not conducted immediately following the first check of the vehicle upon its arrival to the center at 7:42 a.m. The driver of the vehicle parked the vehicle and went inside the building before the second person came out to conduct the second check. The staff member signed the checklist indicating that a second check had been completed, however a staff member did not physically walk through the vehicle. As a result, a four-year-old child was left unattended on the bus for approximately three minutes. 6. Rule 591-1-1-.36(8) restricts a child’s travel time to forty-five minutes on each trip between the Center and destination unless accompanied by the Parent and excluding field trip. During an investigation, it was determined that on December 6, 2023, a four-year-old child traveled for 52 minutes when the child was picked up from their home at 6:50 a.m. and did not arrive to the center until 7:42 a.m.

During an investigation, it was determined that on December 6, 2023, a four-year-old child was left unattended on a vehicle following home transportation. Staff arrived at the facility and the children exited the vehicle. The driver checked the children off on the checklist as they exited the vehicle then backed the bus into the parking space, but .did not conduct a physical first check of the vehicle before exiting. At 7:45 a.m. a staff member who was checking the ratios in the classroom observed the child was not in the classroom. The staff member went out to the vehicle and discovered the child. The child had been left unattended on the vehicle for approximately three minutes.
