    During a Monitoring Visit conducted on December 15, 2025 ; It was determined based on observation that the following hazards were observed: -G-Back 2L: An empty package of wipes was observed to be accessible at the sink area. -H-Back 3L: Hand sanitizer was observed to be accessible near the white board area. -I-4R: A broom and dust pan were observed to the right near the sink. In the hallway between the G-Back 2L Left and H-Back 3L: empty Ziplock and grocery bags were observed accessible in children's cubbies. Also, a bottle of sanitizer was observed on the cubby shelf and accessible near the H-Back 3L classroom.

Further, upon arrival to the center, the consultant observed Staff outside with children playing on a water slide set up in the parking lot and it was not enclosed with a 4 foot high barrier. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. "Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers, and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm’s length away from children thirty-six (36) months of age and younger.

    During an Incident Investigation Closure conducted on April 3, 2025 ; It was determined based on an investigation, that on January 31, 2025, at 10:46 a.m., a one-year-old child sustained an injury to their left thumb when it was closed in a classroom door. The center staff used a chair to prop open the classroom door while the children were transitioning from the playground to the classroom. The staff member walked out to the playground to retrieve another child when a child moved the chair and the one-year-old child's finger got shut in the door. The staff member was not positioned to maximize view of the children.

The vents were observed to be dusty in the boys and girls bathroom as well as the classroom. Also, based upon observation the left sink was not draining. -H-Back 3L: The ceiling in the classroom was observed to be peeling. The vents were observed to be dusty in the classroom. -I-4R: The vent in the classroom was observed to be dusty. Spider webs were observed in the girls bathroom. The top on the faucet was observed to be coming off. -K-2R: The closet between L-1R was observed to be missing a cover for the light switch and there was a hole. The sink in the classroom on the right was not draining. -L-1R: There were two areas on the ceiling observed to be dusty in the classroom. Also, based on observation the faucet near the diaper area was not secured.

    During a complaint investigation conducted on October 12, 2021, it was determined based on investigative findings that on October 1, 2021, the center’s video showed that a staff member was observed grabbing a three- year-old child by the left arm. $499.00

    During a complaint investigation conducted on June 10, 2021, it was determined based on an investigative findings that a staff member inappropriately disciplined a two-year-old child when the staff member roughly removed the child from the table after the child dumped their milk on their plate. The staff member further moved the child to the left side of the classroom resulting in the child falling on the floor.     591-1-1-.32(7) · Rule 591-1-1-.32(7) requires that children be supervised at all times and that the persons supervising in the child care area be alert, able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the chaperones and Students-in-Training, and provide timely attention to the children's actions and needs.

    During a complaint investigation conducted on October 12, 2021, it was determined based on investigative findings that on October 1, 2021, a three-year-old child sustained an injury to the head when the child fell hitting their head on a bookshelf. The staff person in charge of the classroom was seated at the art table organizing an art activity.

    591-1-1-.29(3) · Rule 591-1-1-.29(3) requires that the person-in-charge shall report or cause to be reported to the Department within twenty-four (24) hours or the next work day: any death of a child while in the care of the Center; any serious illness or injury requiring hospitalization or professional medical attention other than first aid of a child while in the care of the Center; any situation when a child in care becomes missing, such as, but not limited to, a child who is left on a vehicle, a child who leaves the building, playground, or property, or a child who is left behind on any trip; any fire; any structural disaster; any emergency situation that requires temporarily relocating children; and any time the program’s operating status changes (i.e., open to closed or temporarily closed and temporarily closed to open).

    During a complaint investigation conducted on October 12, 2021, it was determined that the center failed to report to the Department within 24 hours when, on August 6, 2021, a two-year-old child received professional medical attention after the two-year-old child sustained a cut to the head after hitting their head on a bookcase.
