During a Licensing Study conducted on July 20, 2023 ; It was determined based on observation that in Room A was observed to have three infants, three one- year-olds, four three-year-olds, two four-year- olds, one seven-year-old, one eight-year-old, and one nine-year- old with a ratio of 1:15.

During an Incident Investigation Closure conducted on November 9, 2022 ; It was determined based on staff statement and staff interview this incident occurred when a one-year-old who had just started walking, climbed into a child sized chair and fell from the chair, bruising the area above and below the child's eye. Staff was unable to prevent the injury from occurring. This situation was substantiated. In regard to the required report, self-reported by the center on October 5, 2022, stating on September 15, 2022, the center staff failed to maintain watchful oversight when a one-year-old child fell from climbing up a chair and hit their eyebrow on the edge of a table. Staff were not aware of how the injury occurred. The child was observed by medical professionals and diagnosed with intense bruising. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

Additionally, consultant observed a staff member to leave the eight children unattended in classroom A- Front while the staff member began preparing lunch in the kitchen. Furthermore an infant was observed to be moved to a crib, for napping a B-Back Classroom and left unattended. The staff member left the room to clean the kitchen. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

Additionally, the other staff took one infant outside to the swings for five minutes. Furthermore, an infant and toddler were left napping in classroom B- back classroom and left unattended.
