During a Complaint Closure conducted on October 14, 2025 ; It was determined that on September 5, 2025, three staff members were present at the center until approximately 10 a.m. with 21 children present. Based on sign-in and out and confirmed by the director. the center had three infants, six 1-year- old, four 2-year-old, and eight 3-year-old children present. The center staff indicated that breakfast is served daily at 8 a.m. and based on the staff present and the children present, and staff indicating that children are not mixed, one staff member was present with all children under the age of two during breakfast time, which was a ratio of one staff member to 11 children.
