During an Incident Investigation Closure conducted on October 21, 2022, it was determined that on October 3, 2022, a three-year-old child walked out of the front door of the center unsupervised. The child walked out of the classroom through an opened door, walked to the front door, unnoticed and was able to open the front door and walk out into the parking lot. The child was found and returned to the center by a person not affiliated with the program. Staff were not aware that the child was missing until the child was returned approximately 30 minutes later.

The child sustained an injury that required professional medical attention. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable.
