591-1-1-.08(1) · Rule 591-1-1-.08(1) requires the Center Staff to maintain a file for each child while such child is in care and for one year after that child is no longer enrolled. In order for the file to be complete, the file shall contain the following: child¶s name, birth date, sex, address, living arrangement, name of school if applicable; names of both Parents, home and work addresses, and home and work telephone numbers; name(s) and addresses of the person(s) to whom the child may be released including address, telephone numbers, relationship to child and to Parent(s), and other identifying information; name(s) and telephone number(s) of person(s) to contact in emergencies when the Parent cannot be reached; name and telephone number of the child¶s primary source of health care; and a statement regarding known allergies, physical problems, mental health disorders, intellectual disabilities or developmental disabilities which limit the child¶s participation in the program.

During a Licensing Study conducted on May 21, 2025 ; It was determined based on a review of records that three out of five children enrollment packets were missing the following information: - Child #1: names of Parents, Parent's work telephone number, name and telephone number of the child¶s primary source of health care, and address of the person to whom the child may be released. - Child #2: names of Parents, Parent's work telephone number, name and telephone number of the child¶s primary source of health care, and emergency contact information. - Child #3: names of Parents, Parent's work telephone number, and address of the person to whom the child may be released.

During a Monitoring Visit conducted on September 16, 2025 ; It was determined based on a review of records that Child #1, Child #2, Child #4, and Child #6 did not have the name and telephone number of the child¶s primary source of health care on file, as required. It was further determined that Child #3 did not have an enrollment packet on file on this date.

During a Licensing Study conducted on May 21, 2025 ; It was determined based on observation that 16 children were observed in classroom Room D-5-Afterschool-Right, which was licensed for 11 children.
