During a Complaint Investigation & Monitoring Visit conducted on April 5, 2023 ; It was determined the following were observed accessible to children: Room A- baby wipes and ointment were in an unlocked cabinet under the changing table Room B- baby wipes were on a shelf Room D- bubble solution bottle was on the floor near the sink

During a Licensing Study conducted on October 11, 2022 ; It was determined based on review of records that the parent's work addresses were not documented in three of five records reviewed and addresses for persons to whom the child may be released was not documented in one of five records reviewed. It was further determined that the name and/or telephone number of the child¶s primary source of health care was not documented in two of five records reviewed.

591-1-1-.26(8) · Rule 591-1-1-.26(8) requires climbing and swinging equipment to have a resilient surface beneath the equipment and the fall zone from such equipment must be adequately maintained by the Center to assure continuing resiliency.

During a Licensing Study conducted on October 11, 2022 ; It was determined based on observation that the resilient surfaces under the large play structure and swings measured less than three inches. Six inches of resilient surface was required.
