591-1-1-.36(3)(a-b) · Rule 591-1-1-.36(3)(a-b) requires any Center that provides any type of transportation to obtain two (2) hours of state-approved or state-accepted transportation training, biannually, for the Director and for each person responsible for or who participates in the transportation of children. The training shall include, but is not limited to, a review of the transportation rules, a review of approved transportation forms and procedures, and instruction on the usage and completion of the forms and procedures. This training may be counted as part of the annual training requirements for Staff.

During a Monitoring Visit conducted on October 2, 2025 ; It was determined based on a review of records that Staff #1, Staff #2, and Staff #11 participated in transportation but did not have two hours of state-approved transportation training on file, as required, on this date.

During a Monitoring Visit conducted on October 2, 2025 ; It was determined based on observation that the staff member in classroom Main C ±1st Left entered classroom Main D ±Rear to speak to another staff member for approximately 4 minutes, leaving the children in Classroom Main C ±1st Left unsupervised.

During a Licensing Study conducted on April 7, 2026 ; It was determined based on observation that five school-age children left the playground area, walked into Room F - Down - Front, and were unaccompanied by a staff member for approximately 5 minutes. Center Staff shall place an infant to sleep on the infant¶s back in a crib unless the Center has been provided a physician¶s written statement authorizing another sleep position for that particular infant that includes how the infant shall be placed to sleep and a time frame that the instructions are to be followed. When an infant can easily turn over from back to front and back again, Staff shall continue to put the infant to sleep initially on the infant¶s back but allow the infant to roll over into his or her preferred position and not re-position the infant. Sleepers, sleep sacks and wearable blankets that fit according to the commercial manufacturer¶s guidelines and will not slide up around the infant¶s face may be used when necessary for the comfort of the sleeping infant. Swaddling shall not be used unless the Center has been provided a physician¶s written statement authorizing its use for a particular infant that includes instructions and a time frame for swaddling the infant. Center Staff shall not place objects or allow objects to be placed in or on the crib with an infant such as but not limited to toys, pillows, quilts, comforters, bumper pads, sheepskins, stuffed toys, or other soft items and shall not attach objects or allow objects to be attached to a crib with a sleeping infant, such as, but not limited to, crib gyms, toys, mirrors and mobiles. During a Complaint Investigation & Monitoring Visit conducted on July 22, 2022 ; It was determined based on observation that seven children were observed in classroom A - 1st Right, when classroom A - 1st Right was licensed for six children. It was further determined based on observation that 15 children were observed in classroom D-Rear, when classroom D-Rear was licensed for 11 children. It was also determined based on observation that 11 children were observed in classroom E 2nd left, when classroom E 2nd left was licensed for nine children.

During an Incident Investigation Closure conducted on September 13, 2022 ; It was determined that on April 19, 2022, a three-year-old child was not adequately supervised when they climbed over a fence and exited the playground. The child crossed a ditch and into the road, with heavy traffic flow. Before a staff person could reach the child, a passerby stopped their vehicle and picked up the child. The passerby put the child in their vehicle and pulled the vehicle into the parking lot of the center. The staff person took custody of the child from the passerby. The passerby called law enforcement, who completed an incident report. The child¶s parents were immediately notified. The child was not upset or injured. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During an investigation, it was determined that on August 26, 2022, an eleven-year-old child was not adequately supervised when the center staff did not prevent the eleven-year-old from exiting the facility, for an undetermined period of time. The eleven-year-old child became upset with a staff member because the child did not want to leave the facility to get a haircut. The eleven-year-old child ran out of the door running towards the street. During the incident, the center staff followed the eleven-year-old child out the door and brought the child back into the center. L. Griffin Consulting, LLC 2B.
