During a Licensing Study conducted on August 21, 2025 ; It was determined based on observation that the refrigerator on the left side had a thermometer that read 49 degrees Fahrenheit that posed a hazard.

It was further reported that staff did not see and prevent the child from exiting the building as the child followed a staff member into the parking lot approximately 56 feet away from the facility. No fine is being issued at this time.

During an Incident Investigation & Follow Up conducted on August 25, 2025 ; It was determined based on observation that two 5-year-old children, one 6-year-old child, and one 9-year-old child walked from the front lobby to the Main (G) 5R - GA Pre-K (a) classroom without being supervised. During an investigation, it was determined that on November 25, 2024, staff failed to provide adequate supervision resulting in a two-year-old child exiting the front door of the building. The children were in the front lobby of the center waiting for parents to pick up when the child followed a staff member out of the building. The child was missing and unsupervised for approximately one minute. The child was found by their mother approximately fifty- six feet away from the center.

During a Complaint Investigation Follow Up conducted on October 3, 2023 ; It was determined based on observation the following hazards were accessible in the classrooms. Classroom B - Children had access to stacked chairs. Classroom C - Children had access to stacked chairs. Classroom D - Children had access to lotion, wet wipes(hand wipes), and hand sanitizer. Classroom E - Children had access to air freshener. Classroom F - Children had access to stacked chairs. Classroom G - Children had access to rubber gloves. Classroom J - Children had access to toilet plunger and the closet door was unlocked.

Classroom J- There was a box of latex gloves on top of the toilet to the left, children's pull-ups packaging on the floor on the right side of the bathroom.

Playground D: The white PVC pipes were disconnected and open surrounding the grey and green climbing structure. Additionally, one PVC pipe was disconnected and cracked surrounding the swing set. Additionally, the staff member used the same baby wipe used to clean the child's nose to wipe off the table area of another child as they were eating lunch. A staff member in the second right, two-year-old classroom, did not wash their hands after diapering a child.
