Professional medical attention was obtained. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

There were eight children with one classroom staff present while the kitchen staff delivered the children's plates. During an investigation, it was determined that on March 12, 2025, staff members were not adequately supervising the children on the playground when a 4-year-old child fell from the structure and injured their chin. There were two staff members with 15 children, aged three and four years, present on the playground. At approximately 3:08 p.m., one staff member walked toward the door leading to another classroom and began conversing with a parent. The other staff member was observed seated at a picnic table, facing away from the playground structure, and using a mobile device. At approximately 3:09 p.m., the first staff member left the picnic table and walked toward the center building and left the playground area to retrieve a water bottle from the classroom. As the children gathered in one area of the playground on the slide, the 4-year-old child fell from the top of the playground structure near the slide and landed on the ground. Staff failed to actively supervise and provide watchful oversight to the children on the playground, which resulted in an injury to the 4-year-old child’s chin. The child required professional medical attention.
