During an investigation, it was determined that on July 7, 2025, at 11:22 a.m. a 1-year-old child was injured while in care. The Department determined that the center staff failed to intervene or redirect the children when they pushed an unsecured shelf over onto the leg and foot of the 1-year-old child. The staff member observed the children pushing the shelf around the room and did not physically intervene to stop the children from pushing the shelf around the room. Professional medical attention was required and the 1-year-old child was diagnosed with a broken left leg.

During an investigation, it was determined that on July 7, 2025, at 11:22 a.m. a 1-year-old child was injured while in care. The center did not secure the shelf to the floor, wall, or other equipment to ensure that the children are not injured. The children were able to push the shelf around the classroom and then pushed the shelf over onto a 1-year- old child’s leg. Professional medical attention was required and the 1-year-old child was diagnosed with a broken left leg.

During an investigation, it was determined that on July 7, 2025, at 11:22 a.m. a 1-year-old child was injured while in care. The staff did not promptly contact the parent/guardian of a 1-year-old child when a shelf was pushed over on the child’s left leg and foot. The guardian picked up the 1-year-old child, took them home and observed that the child walked with a limp. The guardian notified the center by leaving a voicemail message. On July 8, 2025, the center reviewed video camera footage and confirmed the incident. Professional medical attention was required and the 1-year-old child was diagnosed with a broken left leg.

During an Incident Investigation/Licensing Study conducted on May 2, 2022 ; It was determined based on a review of records that the fire extinguisher was not recorded to have been checked for April 2021, and the smoke detector was not recorded to have been checked for April 2022. Also, the tornado drills were not recorded to have been completed for December 2021 and the review of Emergency plans were missing for 2021. The last entry recorded was January 20, 2021.
