During an incident investigation, it was determined on August 26, 2024, that a one-year-old child was served fish, which was listed as a prohibited food for the child. There were 11 children, and two staff members in the classroom. The staff member served lunch which consisted of fish sticks, green beans, and mandarin oranges.
