During an investigation, it was determined that on December 4, 2025, at approximately 2:18 p.m. a staff member picked up a 1-year-old child by one arm, causing the child to sustain an arm injury. During naptime, the child tripped over a blanket and fell on the ground. A staff member held the child's hand and tried to pull the child up by one arm, which caused the child's arm to twist. The child received professional medical attention and was diagnosed with nursemaid's elbow.

During an Incident Investigation Closure conducted on June 11, 2024, it was determined that on May 7, 2024, center staff did not intervene or redirect a three-year-old child from continuing their unsafe activities when the child ran back and forth across the PVC pipe playground border. The three-year-old child sustained a head injury after they fell and hit their head on a brick wall. Professional medical attention was required for the injury.

During an Incident Investigation Closure conducted on June 11, 2024, it was determined that on May 7, 2024, the center staff did not notify parents immediately after a three-year-old child showed signs of moderate discomfort after hitting their head while on the playground. The child was observed to cry and sit with the staff for several minutes with an ice pack, but center staff did not notify parents immediately as required. Later in the day the child again demonstrated signs of discomfort, and their parents were contacted at that time. The child required professional medical attention as a result of the incident.
