Additionally, many plastic bags were observed in the cubbies accessible to children. -In classroom Left- front toddler, there was a plunger stored in the bathroom accessible to children. -In classroom Right-back 2 1/2- 3, there were several plastic bags observed in the cubbies accessible to children. -In classroom Right front- 3's, there was a plunger stored accessible in the bathroom. Additionally, there were several plastic bags observed in the cubbies accessible to children.

During a Licensing Study conducted on January 28, 2026 ; It was determined based on observation that the following classrooms had indoor hazards accessible to children: -In classroom Left- front toddler, there were 2 plastic bags stored in the cubbies. -In classroom Right-back 2 1/2- 3, there were 4 plastic bags stored in the cubbies. Additionally, there was an unlocked wooden cabinet in the far left corner of the room near the handwashing sink, which stored Clorox disinfectant wipes, two Lysol disinfectant sprays, and a Clorox disinfectant spray.

During an investigation, it was determined that on November 9, 2022, a staff person released a four-year-old child to an unauthorized person not listed on the child’s enrollment form. The individual picked the child up without the parent’s knowledge. The staff checked the individual’s identification, but did not reference the authorized persons pick up list. Law Enforcement responded and the child was returned to the center and was uninjured.
