For example, toddlers shall not be permitted to swing in swings designed for School-age Children. The outdoor equipment shall be free of lead-based paint, sharp corners and shall be regularly maintained in such a way as to be free of rust and splinters that could pose significant safety hazard to the children. During a Licensing Study conducted on November 13, 2024 ; It was determined based on observation that the "S" hooks on the two bucket seats were greater than 0.04 inches therefore posing a tipping hazard to children.

During a Monitoring Visit conducted on September 2, 2025 ; It was determined based on observation that two of two multicolored caterpillar play structures had multiple areas that were broken or cracked, leaving sharp corners and edges that were potentially hazardous to the children in care. It was further determined that two large metal bolts were protruding from the base of the barrier surrounding the area with the bucket swings and posed a safety hazard to the children in care.

During a Monitoring Visit conducted on September 2, 2025 ; It was determined based on observation that a staff member took seven 2-year-old children to the restroom leaving one staff member in the Main Big Room with nine 3-year-old children and eight 4-year-old children, for a ratio of 1:17. A second staff member was needed to meet the minimum ratio of 1:15 since 3-year-olds made up more than 20% of the group. It was further determined based on observation that upon the Consultant's arrival to the center, only one staff member was present in the Main Big Room with a total of 24 children ages 2 to 4 years, for a ratio of 1:24. Three staff members were needed to meet the minimum ratio of 1:10 since 2-year- olds made up more than 20% of the group. Additionally, only two staff members were present in the Main Big Room with a total of seven 2-year-olds, nine 3-year-olds, and eight 4-year-olds for the duration of the visit, for a ratio of 2:24.
