During a Complaint Closure conducted on December 20, 2021 ; It was determined based on investigative findings that on October 25, 2021, a one-year-old child exited the center as they followed a staff member out of the center's front entrance. The staff member did not see the child exit the building behind them. The child was found in the first parking space of the center's parking lot by a center parent and returned to the center. No professional medical attention was reported. Additionally, it was determined based on investigative findings that on October 25, 2021, a second one-year-old child was not properly transitioned to their classroom at the end of the day. The child was found unsupervised in an unoccupied classroom area at the time of the child's pick up. No professional medical attention was reported. The incident was not self-reported to the Department. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a LS POI Follow Up conducted on May 17, 2022 ; It was determined based on observation that the teacher in the front left classroom walked out of the classroom to open the front door and left three children unsupervised in the classroom. Exhibit "A"

It was determined during a complaint investigation conducted on November 2, 2021, that on October 25, 2021, at or near 5:15 p.m., a one-year-old child followed a staff member out of the building and was not noticed by the staff member. It was reported that the child made it to the first parking space of the center’s parking lot. The child was then brought back inside of the center by a center parent. Additionally, a second one-year-old child, on the same date, could not be immediately located when their parent arrived for pick up. This occurred at or near the time of the first incident. The child was found by a staff member unsupervised in an unoccupied classroom area. The staff member did not make sure that the grouping of children which they were responsible for were properly transitioned and accounted for by the next staff member. Professional medical attention was not reported for either incident. This first incident of a missing child was reported to the Department in a timely manner.

The second incident of a missing child was not self-reported by the center to the Department.
