During an investigation, it was determined that on May 8, 2025, at approximately 3:30 p.m., staff failed to provide watchful oversight when a 2-year-old child sustained an injury while on the playground. Five staff members were supervising twenty-five 2- and 3-year-old children on the playground when a 2-year-old child fell from the side of a toddler slide and sustained an arm injury. Staff Members A, B, and C were positioned in various areas of the playground, while Staff Member D was inside administering first aid to another child. Staff Member E observed the injured child crying but did not witness the fall. Staff assessed the child but were unable to determine the cause of the injury and did not report the incident to the administration. The staff members were not adequately positioned to observe the slide area or respond promptly, resulting in a lack of timely attention and failure to prevent or recognize the injury.

During an investigation, it was determined that on May 8, 2025, the Center failed to notify the parent that a 2-year- old child had fallen from a playground slide and experienced signs of moderate discomfort. At approximately 3:30 p.m., the child fell from the slide and began to cry. The child resumed playing; however, a staff person noticed that the child’s arm was swollen. When the child’s parent arrived at approximately 4:15 p.m., the parent noticed swelling of the child’s arm and notified an administrator. The child received professional medical attention and was diagnosed with an arm injury.

During an Incident Investigation by phone on March 19, 2025, it was determined based on staff statements that on February 21, 2025, at approximately 8:25 a.m. staff failed to provide watchful oversight when a one-year-old child was pushed, pulled, and scratched multiple times on the face.
