During a Complaint Investigation Follow Up conducted on January 17, 2025 ; It was determined based on observation in the toddler one and infant two classrooms, a 1-year-old child was observed eating lunch without a staff seated within arms length away from the child. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Investigation & Monitoring Visit conducted on April 12, 2022 ; It was determined based on observation of Playground B, Classroom H (Two's class) staff member was observed standing in the threshold of the classroom doorway leading to the Playground B , a second staff member from Classroom H was observed on Playground A holding the wooden gate open, sent children towards the classroom path where the staff was standing in the threshold receiving the children. The children were observed being directed inside Classroom H where no staff member was present providing watchful oversight due to the staff standing over the threshold of the classroom doorway. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.
