    591-1-1-.30(1)(a)1. · Rule 591-1-1-.30(1)(a)1. requires that cribs be in good repair and free of hazards. Stack cribs and cribs with drop sides shall not be used.

    During an incident investigation, it was determined that on April 9, 2026, at approximately 4:30p.m. the crib in classroom D:3L had a screw become loose resulting in the mattress collapsing and the infant falling from the crib to the floor. The 6-month-old infant fell through the bottom of the crib onto the floor hitting their head. A staff member was observed patting the back of the 6-month-old infant then the top left side of the crib collapsed causing the infant to roll and slide off the crib mattress onto the floor hitting their face and head. The infant received professional medical attention.

    During an incident investigation, it was observed that on April 13, 2026, eight out of 13 cribs did not meet the Consumer Product Safety Commission (CPSC) and American Society of Testing and Materials International (ATSM) requirements. The cribs were observed in the classroom D:3L with a manufacturer date of December 2010, the requirement states cribs manufacturer date should not be present at the center if older that December 2011.

    591-1-1-.29(3) · Rule 591-1-1-.29(3) requires the Director or designated person-in-charge to report or cause to be reported to the Department within twenty-four (24) hours or the next work day: any death of a child while in the care of the Center; any serious illness or injury requiring hospitalization or professional medical attention other than first aid of a child while in the care of the Center; any situation when a child in care becomes missing, such as, but not limited to, a child who is left on a vehicle, a child who leaves the building, playground, or property, or a child who is left behind on any trip; any fire; any structural disaster; any emergency situation that requires temporarily relocating children; and any time the program’s operating status changes (i.e., open to closed or temporarily closed and temporarily closed to open).

    During an incident investigation, it was determined that on April 9, 2026, center staff failed to report an injury requiring professional medical attention within 24 hours as required by the Department. Center Staff were aware that the 6-month-old received professional medical attention on April 9, 2026 and did not report the incident until April 13, 2026.
