    During a Licensing Study conducted on December 20, 2021 ; It was determined based upon review of records, that staff members #2, #3, #4, #6, #7 and #9 do not have their CPR and First Aid Certification. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

    During a Licensing Study conducted on December 20, 2021 ; It was determined based on observation that the teacher in the 1L classroom was observed to be in the 2L classroom during the virtual visit. Consultant observed the two classrooms to be partially divided by a wall and a low gate, with curtains hanging that blocked the view into the other classroom.
