    During a Complaint Investigation & Monitoring Visit conducted on March 20, 2026 ; It was determined based on observation that 3 of 5 cribs sheets were very loose when the Consultant conducted the pinch and pull test. Exhibit "A"

    During a Licensing Study conducted on July 15, 2025 ; It was determined based on observation that the following hazards were observed in classrooms and were labeled keep out of reach of children: -Classroom D-2nd Left had an unlocked cabinet on the diapering station with Aquaphor, Diaper Paste, Desitin Cream, sunscreen, body wash, Vaseline and baby wipes labeled keep out of reach of children that were accessible to children. Consultant also observed Lysol Spray and Lysol Wipes on the portable sink with a first aid kit accessible to children. -Classroom C-1st Left had Lysol Wipes and latex gloves on the half wall ledge, accessible to children. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.
