591-1-1-.36(7)(c)2. · Rule 591-1-1-.36(7)(c)2. requires that the driver or other designated person shall immediately document in writing, with a check or other mark/symbol to account for each child listed on the passenger transportation checklist each time a child enters and exits the vehicle. The driver or other designated staff person shall document in writing with a different mark/symbol to account for each child listed on the passenger transportation checklist who was not present on the vehicle for any reason. An explanation shall be documented in writing whenever a child is transported to a field trip site but is not present on the return trip to the Center.
During a Licensing Study conducted on January 9, 2025 ; It was determined based on review of transportation records that during the week of December 9th, that during the afternoon route children were missing marks for loading at the school and unloading at the center for the following schools: * From Tucker Elementary on Monday December 9 four out of eight, (4/8) on Tuesday December 10, six out of eight, (6/8) on Thursday December 12, six out of eight (6/8)and Friday December 13, eight out of eight (8/8). * From Morningside Elementary on Tuesday December 9, five out of eight (5/8) on Thursday December 12,seven out of eight (7/8) and Friday December 13, eight out of eight (8/8). In addition on Tuesday December 10, during the morning route, two out of eight were missing a mark for loading on the bus from the center and unloading at school. * From Langston Road Elementary on Tuesday December 10, one out of three (1/3), on Wednesday December 11, one out of three (1/3), on Thursday December 12, three out of three (3/3) and Friday December 13, three out of three (3/3). * From Kings Chapel Elementary on Tuesday December 10, one out of one (1/1) and on Friday December 13, one out of one (1/1). In addition, during the week of December 16th, during the morning and afternoon routes, children were missing marks for loading and unloading on the following days: *Monday December 16, four out of five (4/5) children were missing a mark for loading at the center and unloading at the school. *Tuesday December 17, one out of five (1/5) children were missing a mark for loading at the center and unloading at the school *Thursday December 19, two out of five (2/5) children were missing a mark for loading at the center and unloading at the school **Monday December 16, four out of five (4/5) children were missing a mark for loading at the school and unloading at the center **Tuesday December 17, two out of five (2/5) children were missing a mark for loading at the school and unloading at the center **Wednesday December 18, one out of five (1/5) children were missing a mark for loading at the school and unloading at the center **Thursday December 19, two out of five (2/5) children were missing a mark for loading at the school and unloading at the center.

During a Monitoring Visit conducted on August 21, 2025 ; It was determined based on review of transportation records the following: * On August 20, 2025 during the afternoon bus route from Morningside Elementary to the Facility, seven out of seven children being transported were missing a mark for loading the bus at the school and unloading at the facility or being absent.

The responsible person shall physically walk through the entire vehicle; visually inspect all seat surfaces, under all seats and in all compartments or recesses in the vehicle's interior; and sign the passenger transportation checklist(s), indicating all of the children have exited the vehicle. There shall be continuous watchful oversight of the vehicle between the first check and second check.
