During a Complaint Closure conducted on September 1, 2023; It was determined based on an investigation that on August 9, 2023, during afternoon dismissal, staff was observed to not provide adequate supervision when the staff member did not respond promptly the needs and actions of the children in care. One staff was caring for seven one- year-old children when the staff member had their back to the children for several minutes as children pushed the table and chairs around the area, climbed on chairs, and climbed on top of the table. The staff member walked next to a child that was standing on a chair and did not respond to the child’s actions. The child fell from the chair and remained motionless for several minutes. The staff looked in the direction of the children multiple times and stepped over the child laying on the floor as they passed by but did not respond to the child’s needs. The child sustained a head injury as a result of the fall and professional medical attention was required.

During an investigation, it was determined that on August 9, 2023, center staff failed to provide individual attention to a one-year-old child, who remained lying on the floor for approximately ten minutes after falling from a chair. The child climbed onto the chair and remained standing for several minutes until they fell onto the floor. The child laid on the floor motionless for several minutes as the staff member continued to stand with their back to the children and at one point the staff member stepped over the child as they remained motionless on the floor for several minutes. The staff member did not respond to the child, assess the child for injuries, and provide comfort to the child. The child sustained a head injury as a result of the incident and required professional medical attention.

During an investigation, it was determined that on October 1, 2021, an infant child sustained a burn on the right ankle when a staff member reached for a bottle out of the bottle warmer causing the hot water to leak on the infant. The infant’s foot was observed to be red and peeling. Professional medical attention was obtained.
