The responsible person on the vehicle shall physically walk through the entire vehicle; visually inspect all seat surfaces, under all seats and in all compartments or recesses in the vehicle¶s interior; sign the passenger transportation checklist (s), indicating all of the children have exited the vehicle; and give the passenger transportation checklist(s) to the second designated Staff person.
During a Licensing Study conducted on July 22, 2024 ; It was determined based on review of records that the center did not document the first check of the vehicle (tag # GGM707) after a field trip to Jim Cherry Memorial Planetarium conducted on July 12, 2024 and July 19, 2024. Further review of documentation revealed that the center did not document the first check of the vehicle after a field trip to Yellow River wildlife Sanctuary was completed on July 19, 2024.

The state-approved training hours obtained will count toward required first year training hours. The training must address the following health and safety topics: prevention and control of infectious diseases (including immunizations); prevention of sudden infant death syndrome and use of safe sleeping practices; administration of medication, consistent with standards for parental consent; prevention of and response to emergencies due to food and allergic reactions; building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; prevention of shaken baby syndrome, abusive head trauma and child maltreatment; emergency preparedness and response planning for emergencies resulting from a natural disaster or a human-caused event (such as violence at a child care facility); handling and storage of hazardous materials and the appropriate disposal of bio contaminants; precautions in transporting children; recognition and reporting of child abuse and neglect; and child development.
During a Licensing Study conducted on July 22, 2024 ; It was determined based on review of records staff member #14, date of hire of January 8, 2024, did not complete the required health and safety orientation training within the first 90-days of their date of hire.

591-1-1-.33(5)-591-1-1-.33(5)-91-1-1-.33(5) · Rule 591-1-1-.33(5)-591-1-1-.33(5)-91-1-1-.33(5) requires that every calendar year after the first year of employment, all supervisory and caregiver Personnel, except independent contractors, Students-in-Training and volunteers shall attend ten (10) clock hours of diverse training which is task-focused in on-going health, safety and early childhood or child development related topics and which is offered by an accredited college, university or vocational program or other Department- approved source. The annual ten (10) clock hours of training shall be chosen from the following fields: child development, including discipline, guidance, nutrition, injury control and safety; health, including sanitation, disease control, cleanliness, detection and disposition of illness; child abuse and neglect, including identification and reporting, and meeting the needs of abused and/or neglected children; and business related topics, including parental communication, recordkeeping, etc.; provided however that such business related training shall be limited to no more than two (2) of the required ten (10) clock hours of training. Records of completion of such training shall be maintained, as required by these rules.
During an Incident Investigation/Licensing Study conducted on February 25, 2022 ; It was determined based on review of records that staff member #5, date of hire October 5, 2017, staff member #6, date of hire August 3, 2006, did not complete the required 10 hours of annual training for the year of 2021. $299.00 2.

"Supervision" means that the appropriate number of Staff members are physically present in the area where children are being cared for and are providing watchful oversight to the children, volunteers and Students-in-Training. The persons supervising in the child care area must be alert, positioned to maximize their ability to hear and see the children at all times, and able to respond promptly to the needs and actions of the children being supervised, as well as the actions of the volunteers and Students-in-Training, and provide timely attention to the children's actions and needs. Staff shall be attentive and participating with all children during mealtimes and shall be seated within an arm’s length away from children thirty-six (36) months of age and younger.
During an Incident Investigation Closure conducted on January 21, 2021 ; It was determined based on investigative findings that a six-year-old child was not supervised on January 7, 2021 when the child was left on the center's vehicle unsupervised for approximetely twenty-six minutes following routine after- school transportation.

During a Complaint Investigation by Phone conducted on May 7, 2021 ; It was determined based on an investigation that on April 20, 2021 a four-year-old child was left unsupervised on the playground for approximately three minutes.

During an Incident Investigation/Licensing Study conducted on February 25, 2022 ; It was determined based on observation that in classroom 2R two (2) two-year-old children were sitting at a table during mealtime and the staff member was not sitting within arm's length away from the children. The staff member was observed to be sitting at a separate table with their back to the children, rather than sit closely to the child, as required. It was also observed that a school-age child was not properly supervised as they walked from the director's office back to their class unattended. Exhibit "A"
