It was determined based on an investigation that on March 14, 2024, at 8:53 a.m., center staff did not ensure that classroom equipment was secured when a wooden structure fell onto a two-year-old child. Staff stated they had previously told the children not to climb on the equipment. Video footage showed two children pulling on the equipment and it tipping over onto one of the children. The two-year-old child sustained an injury that required professional medical treatment.
