    During a Monitoring Visit conducted on September 29, 2025 ; It was determined based on observation in Room I/9 , three out of seven shelves were empty with no toys or books available to the children on the empty shelves.

    During a Monitoring Visit conducted on September 29, 2025 ; It was determined based on observation that the threads on the bolts of the gate separating the Infant Toddler Playground from the far Right Playground measured longer than two thread lengths.

    During a Licensing Study conducted on February 9, 2026 ; It was determined based on observation that the threads on the bolts of the gate separating the left back playground from the rear playground measured longer than two thread lengths. It was further determined that the fencing throughout the front left playground, rear left playground, and rear playground were in need of repair due to gaps at the bottom of the fence, the wood fence warping, and disconnected poles on the top of the fencing, posing an entrapment hazard.

Current and valid evidence of the successful completion of such training shall be maintained on the Center¶s premises. The hours obtained completing this certification will not count towards the required annual training hours.

    During a Monitoring Visit conducted on September 29, 2025 ; It was determined based on review of records that center staff#3 (hire date 06/23/2025), Staff#15( hire date 06/17/2025), and Staff#2 (hire date 06/14/2025) who were hired before the 45 day new rule became effective did not obtain their CPR/First Aid Certification within the 90 days. Additionally, staff#14 (hire date 07/14/2025) did not obtain a CPR/First Aid certification within the 45 days as required. 5. B.
