During an Incident Investigation Closure conducted on January 27, 2025 ; It was determined based on an investigation that on December 10, 2024, a one-year-old child and another child pulled a shelf down on top of themselves. Center staff were not alert and failed to provide timely attention to the actions of the children when they were unaware that two children were engaging in unsafe play. Professional medical care was obtained. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Closure conducted on February 26, 2025 ; It was determined based on an investigation, that staff were not positioned to maximize their ability to hear and see the children as well as respond promptly to their needs, when a child pushed another two-year-old child several times and the child fell to the ground. During an incident investigation closure conducted on January 27, 2025, it was determined that adequate supervision was not provided on December 10, 2024, when two 1-year-old children pulled on a low shelf in the classroom, resulting in the shelf falling onto the children. The two children went through the openings of the equipment, when one of the children positioned themself on the side with their feet inside one opening and then pulled the shelf. The staff members did not respond. The other child then positioned themself the same way on the same side of the shelf within the other opening, which caused the shelf to topple over and fall on the children. Additionally, large Lego pieces fell and hit the children. One of the children sustained an injury to the mouth, teeth and gums that required professional medical attention. At the time of the incident, staff were engaged in conversation with each other and were not aware of the children’s activities.

During an incident investigation closure conducted on January 27, 2025, it was determined that the center failed to secure classroom equipment on December 10, 2024, when a playscape shelf in the classroom fell onto two one-year-old children. As the children went through the openings of the equipment, they positioned themselves on the same side with their feet inside the openings and then pulled the shelf, which caused the shelf to topple over and fall on the children. Additionally, large Lego pieces fell and hit the children. One of the children sustained an injury to the mouth that required professional medical attention.

During a licensing study conducted on August 19, 2024, it was determined based on observation that there was unsecure equipment in the following classrooms 4R-Blue Jay, a large screen monitor on top of a low shelf in classroom. In classroom 5R-Road runners, an unsecured computer monitor on a low shelf in the classroom. In classroom 5L-Owls, an unsecured computer monitor was in the circle area of room. In classroom 4L-Falcon’s classroom, an unsecured air purifier on top of plastic storage unit in classroom.
