During an investigation, it was determined that on February 17, 2026, at approximately 4:28 p.m., adequate supervision was not provided when a 2-year-old child fell backwards off a piece of equipment not intended for climbing and sustained a head injury. During outdoor play, both staff members were seated in the playground area. The 2-year-old child climbed on a piece of equipment that measured 2 feet, 11 inches in height and was intended to be used as a musical drum. Neither staff member was aware that the child was climbing on the drum until the child reached the top and sat down. When one of the staff members saw the child at the top of the drum, they called out to the child and stood up. At the same time, the child scooted backward and fell from the equipment to the ground. The child struck the back of their head on the ground and received professional medical attention.

During an investigation, it was determined that on February 17, 2026, at approximately 4:28 p.m., a 2-year-old child sustained a head injury on the playground after falling backwards off a piece of equipment. The child’s parents were not notified immediately as required. Staff members were unsure if the child hit their head during the fall but gave ice to the child for their head as a precaution. The child’s parents were not made aware of the incident until they arrived for pick up at approximately 5:30 p.m. After pick up, the child received professional medical attention for a head injury.

During an investigation and follow-up visit conducted on March 3, 2026, it was determined based on an investigation that on February 10, 2026, a 3-year-old child swallowed a heart-shaped Lego tile, and staff members were not aware of the incident. Staff did not supervise in a way that was appropriate for the individual needs and capabilities of the child.

During a complaint closure conducted on March 24, 2025, it was determined based on an investigation that on February 6, 2025, at approximately 10:50 a.m., a 2-year-old child sustained a jaw injury and the center failed to notify the parent immediately. Professional medical attention was required for the injury. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

It was determined based on an investigation that on February 6, 2025, at approximately 10:50 a.m., a staff member engaged in rough play with a two-year-old child when the staff member put the child over their shoulder and ran. The staff member tripped, and the staff member and child fell on the concrete of the playground. The child sustained an injury to their jaw that required professional medical attention.
