It was determined based on an investigation that on April 17, 2024, at approximately 11:55 a.m., a one-year- old child sustained an eye injury to the left eye, while in the classroom. The one-year-old child stood on a chair that toppled over, resulting in the child falling and hitting their left eye on a shelf. There were two staff members with thirteen one-year-old children in the classroom. At the time of the incident, Staff Member A was inputting notes on an iPad while Staff Member B was engaged with other children in the classroom. Staff were not providing watchful oversight and were unaware of what happened to the one-year-old child. According to video footage, the one-year-old child stood on a child-sized chair, which then toppled over, causing the child to hit their left eye on a shelf. The staff members did not provide watchful oversight, the staff members were observed to be preoccupied with engaging with a small group of children and utilizing an iPad device at the time the one- year-old child stood on a child-sized chair, which then toppled over. Professional medical attention was required, the child received four stitches on their left eyebrow area.

It was determined that on August 21, 2023, at approximately 3:00 p.m. a staff member picked up a child in a manner that caused injury. The staff member pulled the one-year-old child up by their left arm and placed them on their feet. The staff member guided the child to the opposite side of the classroom by the arm to change their diaper and apply sunscreen. After being pulled by the arm, the child was not using their left arm, and was crying. As a result of this action, the child sustained an injury to their elbow. Professional medical care was obtained.

During an investigation, it was determined that based on investigative findings which included interviews, statements, and a review of video footage, on April 13, 2023, a staff person handled a two-year-old child roughly resulting in scratches and bruises under the child’s arms. At approximately 1:30 p.m., there were two staff persons with the two-year-old children during nap time. Both staff persons were seated on the floor among the children. The child began to fuss so the second staff person pulled the child onto their lap. Approximately 10 seconds later, the staff person stood and picked the child up under their arms. The staff person carried the child in that manner, quickly out of the classroom to prevent them from waking the other children. A member of management heard the child crying and the staff person speaking loudly in the hallway and brought the child to the front desk. The child remained at the front desk for approximately 30 minutes before returning to the classroom. The parents of the child informed the center the next day that the child received professional medical treatment as a result of the incident.
