During a Licensing Study conducted on September 18, 2024 ; It was determined based on observation that in classroom C six out of six cribs and in classroom D1 three out of three cribs had loose fitting sheets.

During a Licensing Study conducted on September 18, 2024 ; It was determined based on observation that in classroom A, a piece of baseboard was missing along the wall adject to the covered sink. Additionally, there were stained ceiling tiles in classroom A and one loose ceiling tile in classroom B1. In classroom B1, the classroom sink was lacking caulk where the sink is attached to the wall and the right side of the sink panel was loose. It was further determined by observation that the left toilet in the bathroom of classroom F1 was out of order and the sink in classroom E was labeled out of order and underneath the large viewing window on the left wall, there was a gap between the floor and wall that posed a possible hazard. Additionally, the fan located on the wall was covered in a thick layer of dust. Also, the following classrooms had chipped paint: Room A ±chipping paint located on wall to the right of the door between classrooms A and B Room B1±chipping paint located on the exterior exit door Room C ±chipping paint located on the wall with the window between classrooms, located to the right of the window behind the cribs Room D2±chipping paint located along the wall that has the exterior exit door Room E ±chipping paint located throughout the classroom

During a Licensing Study conducted on May 23, 2024 ; It was determined based on observation on playground A the left side fence measured at approximately 46 inches which did not meet the four foot requirement. Also, the top right side of the fence was detached from the pole for playground B . Additionally, the gate leading to playground B was not secure which allowed children to access playground B which was not in use. Playground C exit gate near the parking lot had a gap measuring approximately four and a half (4 1/2) inches.

During a Licensing Study conducted on September 18, 2024 ; It was determined based on observation that Playground A ,left side, the fence measured approximately 46 inches which did not meet the four foot fencing requirements. Additionally, the top right side of the fence was detached from the pole on playground A and the left side fence was missing end caps for the poles located along the bottom side of the fence. Additionally, there were gaps in the fence wiring on playground A along the left side and Playground C exit gate near the parking lot had a gap measuring approximately four and a half inches.
