591-1-1-.33(5)(a) · Rule 591-1-1-.33(5)(a) requires that every calendar year after the first year of employment, all supervisory and caregiver Personnel, except independent contractors, Students-in-Training and volunteers, shall attend ten (10) clock hours of diverse training which is offered by an accredited college, university or vocational program or other Department-approved source.
During a Licensing Study conducted on October 28, 2025 ; It was determined based on a review of records that staff member #10 did not have 10 hours of annual training for the 2024 year.

During a Monitoring Visit conducted on June 11, 2025 ; It was determined based on observation that these hazards were present: Right playground -- front left corner next to the sidewalk was washed out and a hole was present that was 12 x 7 inches and the concrete was broken in two areas of the sidewalk in the same area and sharp pieces were present. Back playground -- weeds and overgrowth were present along the left side of the chain link fence and a weed grew through the privacy fence in the same area.

Back Playground- concrete was observed to not be flush around the upper sidewalk, and around the bottom area where the basketball goal was located in two areas that posed a tripping hazard. Sweetgum Tree seadpods, limbs and exposed tree roots (not sprayed) were present that posed hazards. Walking to the playground, outside the Front Left Class, there was a water hose was present.

Center Staff shall place an infant to sleep on the infant¶s back in a crib unless the Center has been provided a physician¶s written statement authorizing another sleep position for that particular infant that includes how the infant shall be placed to sleep and a time frame that the instructions are to be followed. When an infant can easily turn over from back to front and back again, Staff shall continue to put the infant to sleep initially on the infant¶s back but allow the infant to roll over into his or her preferred position and not re-position the infant. Sleepers, sleep sacks and wearable blankets that fit according to the commercial manufacturer¶s guidelines and will not slide up around the infant¶s face may be used when necessary for the comfort of the sleeping infant. Swaddling shall not be used unless the Center has been provided a physician¶s written statement authorizing its use for a particular infant that includes instructions and a time frame for swaddling the infant. Center Staff shall not place objects or allow objects to be placed in or on the crib with an infant such as but not limited to toys, pillows, quilts, comforters, bumper pads, sheepskins, stuffed toys, or other soft items and shall not attach objects or allow objects to be attached to a crib with a sleeping infant, such as, but not limited to, crib gyms, toys, mirrors and mobiles.
During a Monitoring Visit conducted on June 11, 2025 ; It was determined based on observation that an infant was asleep on their back and was dressed in an unapproved sleeper that had a head covering that was soft in nature.

During a Monitoring Visit conducted on March 26, 2026 ; It was determined based on observation that a child in the Third Right Classroom was observed to be sleeping in a crib with a bib attached to their neck. It was further determined when the staff was asked how children are placed down to sleep it was stated "on their stomach". 5. During a Licensing Study conducted on October 28, 2025 ; It was determined based on a review of records that staff member #13 was alone with children in the 2nd Right Classroom and had only Adult first aid and CPR training on file. 5. B. 5XOH E UHTXLUHVWKDWWKHUHPXVWDOZD\VEHRQHVWDIISHUVRQSUHVHQWLQHDFK classroom where children are present that has current and valid pediatric cardiopulmonary resuscitation (CPR) and pediatric first aid training. During a Monitoring Visit conducted on March 26, 2026 ; It was determined based on a review of records that staff member #5 was alone with children in the 2nd Right Classroom and did not have first aid and CPR training.

591-1-1-.14(2) · Rule 591-1-1-.14(2) requires a Staff member who is trained in CPR and first aid to be on the premises and on any field trip whenever any child is present. In addition, Staff who provide direct care to children must satisfactorily obtain certification in first aid and CPR by December 29, 2016 if employed prior to September 30, 2016 and within 90 days of their hire date if employed after September 30, 2016.
During a Monitoring Visit conducted on June 11, 2025 ; It was determined based on a review of records that two staff members did not obtain first aid and CPR within 90 days of their hire date. 6. B. Rule Previously Cited: 591-1-1-.14(1)(a) requires all Staff who provide direct care to children must obtain certification in a biennial training program in pediatric cardiopulmonary resuscitation (CPR) and a triennial training program in pediatric first aid within the first 45 days of employment. Current and valid evidence of the successful completion of such training shall be maintained on the Center¶s premises. The hours obtained completing this certification will not count towards the required annual training hours. During a Monitoring Visit conducted on March 26, 2026 ; It was determined based on a review of records that staff member's #1 and #13 did not have first aid for pediatric or children related.

During an investigation, it was determined that on June 6, 2024, a staff member moved a chair from under a seven- year-old child while playing a game resulting in the child falling and hitting their head on a shelf. The staff member told the children they needed to sit down at the table and the child was hoovering over the chair when the staff member pulled the chair from under the child and the child fell onto a shelf injuring their head. The injury required professional medical attention.
