During an investigation, it was determined that on August 7, 2024, a staff member discovered that a seven-year- old child was left unattended on a vehicle for 21 minutes. The seven-year-old child boarded the vehicle at 7:51 a.m. and fell asleep during transportation. The vehicle returned to the center at approximately 8:15 a.m. and the driver parked and exited the vehicle. The child awoke and found themselves alone on the vehicle. The child exited the vehicle on their own at 8:36 a.m. and was unattended on the vehicle for 21 minutes.

During an investigation, it was determined that on August 7, 2024, the staff member completed the transportation checklist, but failed to account for each child as they exited the vehicle, which resulted in a seven-year-old child being left unsupervised on the vehicle for 21 minutes. The child fell asleep on the vehicle during transit. The child exited the vehicle on their own and entered the center.

The driver parked the vehicle and went inside the building, which resulted in a seven-year-old child being left unsupervised on the vehicle for 21 minutes. 5. Rule 591-1-1 36(7)(d)2 requires that the second designated Staff person conduct a check of the vehicle immediately upon the completion of the first check of the vehicle. The responsible person shall physically walk through the entire vehicle; visually inspect all seat surfaces, under all seats and in all compartments or recesses in the vehicle's interior; and sign the passenger transportation checklist(s), indicating all of the children have exited the vehicle. There shall be continuous watchful oversight of the vehicle between the first check and second check. During an investigation, it was determined that on August 7, 2024, the second check of the center vehicle was not completed at 8:15 a.m. The driver returned to the center, parked the vehicle, entered the building and no second person conducted the second check, which resulted in a seven-year-old child being left unsupervised on the vehicle for 21 minutes.

While in the classroom during free play, the one-year-old child sustained a swollen and bruised right hand due to the hand being closed and caught in the door frame. Based on video footage, the one-year-old child’s hand was caught in the door as the parent attempted to enter the classroom. The staff member did not redirect the one-year-old child to move away from the door when the door was being closed. Medical attention was obtained.

The staff member noticed that the child was missing approximately one minute later. Center staff searched for the child in the center and on the playground. The child crossed a two-lane road and walked into a Dollar General store approximately 0.2 miles away from the center. The child went straight to the toy section. A bystander followed the child into the store and discussed the incident with the Dollar Store manager. The store manager took the child to the office while the bystander went to center to notify center staff of the child's whereabouts. The child was returned to the center at approximately 10:20 a.m.
