    591-1-1-.15(2) · Rule 591-1-1-.15(2) requires that a signed written feeding plan for children less than one (1) year of age shall be obtained from Parent(s) and that instructions from the Parent(s) shall be updated regularly as new foods are added or other dietary changes are made. The feeding plan shall be posted in the child's assigned room and must include the child's feeding schedule, the amount of formula or breast milk to be given, instructions for the introduction of solid foods, the amount of food to be given and notation of any type(s) of commercially premixed formula which may not be used in an emergency because of food allergies.

    During an investigation, it was determined that on October 28, 2024, at approximately 10:15 a.m., an eleven-month- old child was mistakenly given another child's bottle of formula instead of their own bottle of breast milk due to staff failing to ensure the children had their correct bottles. Three staff members were caring for nine infants when multiple bottles were warmed and prepared to be given to children. A staff member gave each child a bottle in their chairs and a few moments later a staff member noticed that the child was no longer drinking the bottle. At that time, the staff determined that the child had been given the wrong bottle. The child began to show signs of a serious allergic reaction and emergency services were called. As a result of the incident, the child had a severe allergic reaction that required professional medical attention.
