During an investigation, it was determined that on February 17, 2026, a 5-year-old child sustained an injury due to hazardous fencing material. During outdoor play, the child walked into a playground fence. Sharp metal ends at the top of the fence caused a laceration on the child’s lower right eye and cheek. The child received professional medical attention.

During an Incident Investigation Closure conducted on March 19, 2026 ; It was determined based on an investigation that on February 17, 2026, the Center failed to keep the fence on the playground in good repair, and a 5-year-old child sustained a laceration to their lower right eye and cheek after walking into the fence, as a result of a sharp area sticking out at the top. Professional medical attention was obtained for the child.

During a Complaint Closure conducted on July 22, 2025 ; It was determined, based on an investigation, that on June 4, 2025, at 4:36 p.m., the center staff failed to provide watchful oversight when a 3-year-old child was running in Classroom G L 2R and sustained a head injury. The teacher was positioned at the front of the classroom and did not see the child running around a table. The child hit the side of their head on a chair and fell. The teacher was alerted by the child¶s cry when they turned around and responded to the incident. The child required professional medical attention, and they received stitches to their injury. As required by the Department, the center staff was not strategically positioned to see the child that was running in the classroom to prevent injury. An adverse action was previously issued for this rule violation. This rule violation is no longer appealable. No fine is being issued at this time.

During a Complaint Closure conducted on July 22, 2025; It was determined based on an investigation that on June 4, 2025, children were not adequately supervised when a 3-year-old child was running in the classroom and fell, hitting a chair. There was one staff member, and 12 children present in the classroom at the time of the incident. The staff member was positioned at the front of the classroom in the process of preparing and distributing snack. . While engaged in this task, the staff member was turned away from the children and did not observe the child running around a table with two other children who had previously been verbally redirected to stop running. The staff member only became aware of the incident after they heard the child cry out. Upon turning around, the staff member observed that the child had fallen and hit their head on a chair. According to the staff statement, the staff member verbally redirected two of the children, however, did not observe the 3-year-old child running around the table. Staff failed to actively supervise and provide watchful oversight to the children while in the classroom, which resulted in the 3-year-old child being injured. The child required professional medical attention and received stitches to their right eyebrow as a result of the incident.
